| Phos | Oct 24, 2007 6:28am | As far as I know Fentanyl has no active metabolites, so clearance issues shouldn't arise with it if you can tolerate it. Can get it in a patch (Duragesic) that delivers a consistent amount over 2-3 days (they even have a lower starting dose now for those that are not opioid tolerant), and there are now two forms of fentanyl breakthrough (neither is cheap) as well. Methadone also doesn't have renal clearance issues, but it wouldn't be my first choice unless its neuropathic pain. It interacts poorly with a lot of antibiotics, and has an incredibly variable half life - so its slow to safely adjust to the proper dose.
If the pain is only occasional but comes on severe and suddenly - Actiq (fentanyl in a loli-pop form) is an excellent choice. Hope your insurance covers it or will make an exception, its *not* cheap. Its an extremely fast delivery system, and a very potent pain drug.
Just some ideas for you to discuss with your doctor or pain management physician. |
